Supervisors will provide the most compassionate, least restrictive, community based care for individuals to ensure their highest possible quality of life.MINIMUM EDUCATION AND EXPERIENCEThe employee must have a four-year degree in the Human Services field and/or at least 5 years of direct experience with I/DD individuals.MINIMUM SKILLS, KNOWLEDGE AND ABILITIESThe employee must have:Knowledge of administrative and clerical procedures and systems such as word processing, outlook, excel, managing files and records, designing forms, and other office procedures and terminology.Knowledge of the medical field, which includes working directly with doctors, nurses, therapist, and pharmacist.Demonstrates organizational, time management, managerial, and leadership skills.Ability to be flexible!Crisis InterventionTOVA, First Aid and CPR, Medication Management, Human Rights/ID Waiver, Emergency Preparedness, Person Centered Thinking TrainingsDBHDS StandardsOffice of Human Rights StandardsDMAS regulationsDUTIES AND RESPONSIBILITIESSupervisionServe as requested on employee selection team, evaluating prospective staff members and making recommendations as needed.Orient new direct care staff to Day Support and their job duties at Day Support.Provide ongoing training and support to GNDS staff.Shall familiarize one’s self with GNCS policies and procedures and be able to address staff-related concerns in an informal manner.Secure staff to cover vacancies in schedule due to staff absences and no-shows. In the event this is not possible, responsibility for coverage lies with Day Support Supervisor if the Team Leader is unavailable.Conduct and document monthly supervision meetings with direct support professionals.Complete annual employee performance evaluations for all direct reports.Responsible for ensuring staff are knowledgeable on the Plan for Supports and support instructions.DocumentationCorrectly Purge each Individuals Chart, MAR, and Support Book each monthEnsure all paper work is in the correct section of each individual’s book after the previous month has been purged outAudit purged paperwork monthly to ensure all has been completed correctly.Audit annual paperwork monthly to ensure that all paperwork is present; contact Support Coordinators and Residential for paperwork needed.Physician’s Orders are in the medical visit books, transportation books, and MAR’sConduct daily audit of Individuals charts and MARContact staff to make corrections as needed.Fill out Time Cards on a weekly basisActivity Calendars complete and submitted to your Manager on the 20th of each monthComplete and submit Person Centered Reviews to the Manager on the 21st of each month.Fax Person Centered Reviews to the Support Coordinator on the day they are due.Two fire drills completed monthlyCheck Day Support phone messages on a daily basis.Physical Environment inspection sheet is completed quarterlyFire extinguisher inspection, fire alarm inspection, and water check completed monthly.Update Individuals paper work when neededConduct weekly phone calls to Support Coordinators, Families, Legal Guardians, etc.Complete phone communication logs during important phone callsSend out emails to administration when there is incidents or behaviorsAll paperwork must be completed prior to leaving shiftProvide supervision of each individual and document daily on Support Logs, Nutrition Logs, and any other specialized documentAssure new employees have been through the required trainings and been trained on each section of the individuals chartMake sure new employee paperwork has been completed and sent to the Main Office.Responsible for the overall physical appearance of the charts (inside and out).MedicalObtain updated information on individuals as neededAttend visits to the ER or transport individuals safely to the hospital as neededAdvocate for the individuals during the appointment and completely fill out the medical visit formHave the doctor sign off on the medical visit formUpdate Person Center Reviews after the doctor’s appointments using the medical visit form.Complete any special instructions in the MAR such as weight, blood sugar/glucose, blood pressure, and pulse.Write in new prescriptions from the doctor on the MAR and PODiscontinue medications on the MAR and POCheck bubble packs and cards on a daily basis for expired medication and medication that was not administeredOverall cleanliness of the medication closetEducate individuals on new medicationResponsible for the overall physical appearance of the charts (inside and out).Go through the monthly medication from Residential to ensure everything the medication is accounted for and the MAR’s and PO’s are printed correctly.Contact Residential for PRN or routine medications needed during Day Support.Physical EnvironmentDaily checks of the facility for cleanliness that meet Medicaid requirementsThe exterior and interior of the individuals’ vans are cleanGas is filled in van when needed.Routine maintenance on the vanAir vents are checked weekly and changed out monthlyDay Support is fully stocked with groceries, cleaning supplies, office supplies, and activity supplies.Quality Food is in the refrigeratorFood is labeled and datedSupervisor’s office is neat, clean and organizedIndividual SupportSupport individuals with Plan of Support goals and objectivesProvide on shift coverage in lieu of a call out, behavior, or medical emergencyResponsible for responding to incidents after hoursProvide safety and support while participating in daily activitiesMenus are properly being implementedEnsure that all individuals have neat, clean and weather-appropriate attire at all timesMeals are prepared according to doctor’s or dietician’s ordersCall transportation provider when rides arrive late and facilitate arrangement of alternative transportation when needed.”Work Remotely * NoJob Type: Full-timePay: $45,000.00 – $52,000.00 per yearBenefits:401(k)401(k) matchingDental insuranceEmployee assistance programFlexible spending accountHealth insuranceHealth savings accountLife insurancePaid time offRetirement planVision insuranceSchedule:Monday to FridayEducation:High school or equivalent (Required)Experience:working with ID/DD services: 5 years (Preferred)License/Certification:Driver’s License (Preferred)Work Location: One location |
