| Job Description : | “Enhancing the lives of those we serve every day” Essential Duties: Population Health Manager’s primary purpose is creating, managing and tracking clinical programs and projects. The Population Health Manager will help advance SuperCare’s key clinical objectives, support referral and payer partners via established committees and by creating new work teams. This role also facilitates the development of internal and external reports and delivery of data as needed to support and monitor the action plans to accomplish the Triple Aim: 1) Improve customer experience, 2) Improve quality of care and patient outcomes, 3) Reduce healthcare costs. Key Functions: Population Health Manager works under the guidance of the Director of Respiratory Care and Director of Clinical Services to accomplish the following initiatives: Develop clinical programs for specific patient populations Recommend and implement technology solutions, software updates for clinical program management Design work plans, workflows and processes by which these programs would be managed Communicate and help develop necessary policies and procedures to aide in execution Collaborate with Technical Training teams on creation and rollout of educational materials Facilitate and track program pilots as deemed necessary Design and analyze performance and outcomes reporting for clinical programs Track progress of action plans, present outcomes to peers, the leadership and outside partners Monitor program integrity and efficacy through routine audits, report findings to key stakeholders Responsible for additional population health management tasks and initiatives defined by management Knowledge / Skills / Abilities: Very strong understanding of clinical programs and pop health initiatives such as HEDIS, STARS, P4P Proven track record of clinical program rollouts for small and large populations Extensive experience with various patient and population health management software Advanced MS Office, various reporting tools, and presentation skills Excellent written and verbal communication skills Minimum Qualifications: Education: Bachelor’s Degree in Public Health, Healthcare Admin, Social Work or equivalent Experience: Minimum of 5 years’ experience in Population Health or Program Management Desired: Degree and license in a clinical discipline Physical Requirements: Benefits: Savannah Stephenson – HR Talen AcquisitionLinkedIn https://www.linkedin.com/in/sa… (https://www.linkedin.com/in/savannah-stephenson-044598159/) |
