||The Director of Modality Education supports the growth of home dialysis and patient admissions by creating the infrastructure, processes, and tools to provide education for optimal management of advanced CKD, modality education, and care coordination to patients and families, physicians, community members, USRC Staff and Hospital partners. This role is responsible for creating new capabilities to effectively deliver Modality Education and help patients either in their journey of starting dialysis or a smooth transition between modalities. These tools will be used by our fast-growing teams of Modality Educators / Care Coordinators, Social Workers, and Home Nurses. This individual will work closely with our Regional Operations Leads, Corporate Home Therapies team, and Clinical Services to build and execute the programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned.
Create the Modality Education and Care Coordination operations and infrastructure at U.S. Renal Care.
Oversees the operations of the Kidney Care Operations programs for U.S. Renal Care, to ensure every patient along with their care partner receives comprehensive education on modality options.
Responsible for overseeing the continuum of care coordination for advanced CKD patient including but not limited to, access placement, financial concerns, and coordination of admission at USRC facility.
Partner with Medical Office and Clinical Services to continuously improve educational content. Ensure market leading delivery of content in the field. Build a brand for Kidney Care Options education program in the market.
Launching and operating the on-boarding and training curriculum for Modality Education, including programs targeted at Modality Educators / Care Coordinator, Social Workers, Home Nurses, and other roles that may be identified to deliver Modality Education.
Establish operating routines and processes for delivering Modality Education, including both individual and group class settings. Identify any investment in technology to increase operational efficiency.
Build toolkits for U.S. Renal Operations Teams to develop deeper relationships with Physician Partners and Hospital Partners through Modality Education services.
Create reporting and tracking tools to continuously enhance Modality Education operations. Create tools for regional leaders to hold modality educators and care coordinators accountable to results.
Partner with Home Growth team to develop roadmap for CKD care coordination.
Upholds management goals of corporation by leading staff in team concepts and promoting a team effort.
Maintain effective personnel management and employee relations, including evaluating the performance of all personnel; approving and submitting all hours worked and counseling and disciplining employees.
Effectively communicates expectations; accepts accountability and holds others accountable for performance.
Regular and reliable attendance is required for the job.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Bachelor’s degree or equivalent work experience.
Five (5) plus years of experience in a healthcare organization working with patients and/or physicians in a clinical setting.
Knowledge of nephrology, End-Stage Renal Disease (ESRD) or similar field required.
Experience in educating patients and staff preferred.
Sales experience is a plus.
Strong verbal and written communication skills.
Excellent customer service skills and ability to communicate effectively, both verbally and in writing.
Must have intermediate computer skills, including Microsoft Office (Word, Excel, Power Point, Outlook and web based tools); proficiency in USRC applications required within 90 days of hire.
